You had a liver ultrasound. The results came back. And the report reads like it was written in another language: "hepatic steatosis with increased echogenicity," "heterogeneous echotexture suggestive of parenchymal disease," "no focal hepatic lesion," "mild splenomegaly," "patent portal vein with hepatopetal flow." You stare at it, wondering whether you should be relieved or terrified — and your follow-up appointment isn't for two weeks.

This guide translates every common term in a liver ultrasound report into plain language — so you can understand what was found (and what wasn't) before your next appointment.


What a liver ultrasound shows

Ultrasound uses sound waves to create images of your organs. It's painless, radiation-free, and widely available. For liver patients, ultrasound evaluates liver size and shape, liver texture (echogenicity — how the tissue reflects sound waves), presence of fatty changes (steatosis), focal lesions (masses, nodules, cysts), blood flow through liver and portal vessels, spleen size, ascites (free fluid in the abdomen), and bile duct diameter (blockage vs normal flow).

Ultrasound has limitations: it's less detailed than CT or MRI, operator-dependent (skill of the sonographer matters), limited by body habitus (harder to image in larger patients), and can't stage fibrosis reliably (that's what FibroScan is for). But it's the first-line imaging for liver disease and the standard tool for HCC screening.

"Normal hepatic echotexture"

The liver tissue looks normal — uniform, smooth texture

Good news. No evidence of fatty changes, fibrosis, or other abnormality on this imaging modality.

"Increased echogenicity" or "hyperechoic liver"

The liver reflects more sound waves than normal — appears brighter on the image

Most commonly indicates fatty liver (steatosis). Fat in liver cells makes the tissue more reflective. The brighter the liver compared to the kidney (used as reference), the more fat is present.

"Hepatic steatosis"

Fat in the liver — confirmed by the ultrasound pattern

This is NAFLD on imaging. Ultrasound can detect moderate-to-severe steatosis but may miss mild fat. Doesn't tell you whether you have simple fat (benign) or NASH (inflammatory — needs further evaluation).

"Heterogeneous echotexture"

The liver texture is uneven — patchy, coarse, or irregular rather than smooth and uniform

Suggests chronic liver disease — fibrosis, cirrhosis, or diffuse parenchymal disease. The scar tissue disrupts the normally smooth texture. This finding warrants further evaluation with FibroScan or biopsy to determine the degree of fibrosis.

"Coarsened echotexture"

Similar to heterogeneous — the texture is rougher than normal

Often indicates chronic hepatitis or early cirrhosis. The liver surface may also appear irregular or nodular.

"Nodular liver surface"

The normally smooth liver contour appears bumpy or irregular

Strongly suggestive of cirrhosis. The nodularity represents regenerative nodules surrounded by scar tissue — the architectural hallmark of cirrhosis.

"Hepatomegaly"

Enlarged liver (typically >15.5 cm in the midclavicular line)

Can be caused by fatty liver, inflammation, congestion (heart failure), infiltrative diseases, or early cirrhosis. Context matters — your doctor will interpret this alongside your labs and symptoms.

"Shrunken liver" or "small liver"

Liver is smaller than normal

Suggests advanced cirrhosis — significant tissue loss from scarring. A small, nodular liver is a classic imaging finding in end-stage liver disease.

Focal findings (spots, masses, and lesions)

"No focal hepatic lesion"

No masses, nodules, or abnormal spots were seen

Reassuring — no evidence of tumors or cysts on this exam. This is what you want to see on HCC screening.

"Simple hepatic cyst"

A fluid-filled sac inside the liver with thin walls and clear contents

Benign — almost always harmless. Simple cysts are extremely common (found in ~5% of the population), don't become cancer, and usually require no follow-up unless they're very large or causing symptoms.

"Hepatic hemangioma"

A benign blood vessel tumor in the liver

Benign. The most common solid liver lesion. Not cancer. Not pre-cancerous. Usually requires no treatment unless very large (>10 cm). Found incidentally in 5–7% of adults.

"Echogenic focus" or "hyperechoic lesion"

A bright spot in the liver

Could be a hemangioma, focal fat deposition, calcification, or (less likely) a tumor. Additional imaging (CT or MRI with contrast) is typically recommended to characterize it — especially in cirrhosis patients where HCC must be excluded.

"Hypoechoic lesion" or "hypoechoic mass"

A dark spot in the liver — reflects fewer sound waves than surrounding tissue

Requires further investigation. In cirrhosis patients, this is suspicious for HCC until proven otherwise. Cross-sectional imaging (multiphase CT or MRI) is the next step. In non-cirrhotic patients, differential includes benign lesions (focal nodular hyperplasia, adenoma) alongside malignant possibilities.

"Indeterminate lesion"

A spot that can't be clearly classified as benign or malignant on ultrasound

Requires further imaging — CT or MRI with contrast to characterize. Don't panic — many indeterminate lesions turn out to be benign. But in cirrhosis, all indeterminate lesions must be investigated promptly.

Blood flow and vessels

"Patent portal vein with hepatopetal flow"

The portal vein is open and blood is flowing in the normal direction (toward the liver)

Normal. Good news.

"Hepatofugal flow"

Blood in the portal vein is flowing AWAY from the liver (reversed)

Abnormal — indicates severe portal hypertension. The pressure in the liver is so high that blood is being pushed backward. Significant clinical finding.

"Portal vein thrombosis"

A blood clot in the portal vein — partially or completely blocking blood flow

Serious finding that affects liver function, portal pressure, and transplant planning. Requires anticoagulation evaluation and hepatologist management.

"Dilated portal vein"

Portal vein diameter >13 mm

Suggests portal hypertension — increased pressure is causing the vein to widen. Often seen alongside splenomegaly.

Spleen and fluid

"Splenomegaly"

Enlarged spleen (>12 cm)

In liver disease context: indicates portal hypertension. Read the full guide.

"Ascites" or "free fluid in the abdomen"

Fluid visible in the peritoneal cavity

Confirms ascites. May be described as "trace," "small," "moderate," or "large" volume. Even trace ascites in a cirrhosis patient is clinically significant.

"No ascites"

No free fluid detected

Reassuring — no evidence of fluid accumulation at the time of the scan.

Bile ducts

"No intrahepatic biliary dilatation"

Bile ducts inside the liver are normal size — not blocked

Good — no evidence of bile duct obstruction.

"Dilated bile ducts" or "biliary dilatation"

Bile ducts are wider than normal — suggesting downstream blockage

Requires investigation — possible gallstone obstruction, stricture, or tumor compressing the bile duct. MRCP or ERCP may be needed.

"Common bile duct measures X mm"

The diameter of the main bile duct (normal: <6 mm; up to 8 mm after cholecystectomy)

Measurement for tracking. Dilation (>6–8 mm) suggests possible obstruction.


What to do with your results

  • Log your ultrasound results in LiverTracker — date, key findings, and any follow-up recommendations.

  • Don't self-diagnose from terminology. "Heterogeneous echotexture" doesn't automatically mean cirrhosis. "Echogenic focus" doesn't automatically mean cancer. Context matters — your hepatologist interprets these findings alongside your labs, symptoms, and medical history.

  • Ask your doctor to explain findings you don't understand. "What does 'coarsened echotexture' mean for my specific situation?" is a more useful question than "Is it bad?"

  • Track changes over serial exams. A single ultrasound is a snapshot. Serial ultrasounds (compared over time) reveal trends — stable findings are reassuring; new or worsening findings warrant attention.   • If a lesion is found — follow up promptly. In cirrhosis, any new lesion requires characterization with cross-sectional imaging (CT or MRI). Don't wait. The difference between a curable early HCC and an advanced one is measured in the months between detection and follow-up.

My ultrasound says "fatty liver." Should I be worried?

Fatty liver (steatosis) on ultrasound is extremely common — affecting 30% of adults. By itself, it's not an emergency. My ultrasound found a cyst. Is it cancer?

A simple cyst (thin-walled, clear fluid) is almost always benign — not cancer, not pre-cancerous. Simple cysts are found incidentally in ~5% of adults and require no treatment. Complex cysts (thick walls, internal septations, solid components) are different — they require further imaging to characterize. Your radiologist's description ("simple" vs "complex") determines the next step.

How often should I get a liver ultrasound?

For HCC screening: every 6 months if you have cirrhosis. For monitoring steatosis or chronic liver disease: typically every 6–12 months, depending on your hepatologist's assessment. For a one-time finding that was resolved: no routine repeat unless symptoms recur.

My report says "limited study due to body habitus." What does that mean?

It means your body size (usually obesity or large body frame) made it difficult for the ultrasound waves to penetrate deeply enough for clear images. This is a common limitation — it doesn't mean anything is wrong, but it means the study may have missed findings that would be visible in a thinner patient. Your doctor may recommend CT, MRI, or MR elastography as alternatives that aren't limited by body habitus.
